Please create a temporary Login ID and PIN that you will use for the Application for Enrollment.
Please remember your Login ID and PIN in the event you cannot finish the Application for Enrollment and need to finish later.
1. Your login ID can be up to nine alphanumeric characters (9 letters or numbers) with no spaces, dashes or special symbols.
2. Your PIN must be six numeric characters (6 numbers), with no spaces, dashes or special symbols.
3. Re-enter your PIN to verify it and click on Login.
